Waiver and Release of Liability:

I, the undersigned, as the parent or legal guardian of the above-named child, hereby give my full consent and approval for my child to participate in the Grandkid Investigator Summer Challenge, which will be held on a private online platform forum and in person.

I understand that participation in this program involves various activities that may include, but are not limited to, online discussions, virtual educational projects, outdoor games, field trips, and physical exercises. I acknowledge that there are inherent risks associated with these activities, including the risk of personal injury or harm, as well as potential risks associated with online participation.

I hereby release, discharge, and hold harmless The Value of Wrinkles LLC,, its employees, volunteers, and any other representatives from any and all claims, demands, actions, or causes of action arising out of or in connection with my child's participation in the Grandkid Investigator Summer Challenge, including but not limited to any injuries, damages, or losses that may occur as a result of my child's participation in the program, whether online or in person.

Medical Authorization (for in person events)::

In the event of an emergency, I hereby authorize the staff of the Grandkid Investigator Summer Challenge to obtain necessary medical treatment for my child. I understand that reasonable efforts will be made to contact me prior to obtaining any medical treatment. I agree to be responsible for any medical expenses incurred as a result of my child's participation in the program.

Photo and Media Release:

I give permission for The Value of Wrinkles LLC to take photographs and/or videos of my child during the program. I understand that these photographs and/or videos may be used for promotional purposes, including but not limited to brochures, social media, and the organization’s website. Furthermore, I understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

Online Participation Consent:

I acknowledge that my child will be participating in activities on a private online platform forum. I understand the importance of online safety and agree to supervise my child’s use of the platform. I consent to my child’s participation in the online components of the program and understand that measures will be taken to ensure a safe and secure online environment.

Code of Conduct:

I acknowledge that my child is expected to adhere to the rules and guidelines set forth by the Grandkid Investigator Summer Challenge, both online and in person. I understand that any disruptive or inappropriate behavior may result in my child being dismissed from the program without a refund.

Parent/Guardian Acknowledgment:

By signing below, I acknowledge that I have read and understood this waiver and consent form in its entirety. I agree to the terms and conditions stated above and give permission for my child to participate in the Grandkid Investigator Summer Challenge.
